The Park District is maintaining touchless transactions.
- Confirm you have a Household Account created. If not, include the Household Form.
- Fill out and return the Registration Form via the Park District mailbox or email to Michelle Koegel, Early Childhood Recreation Supervisor, michellek@clarendonhillsparkdistrict.org
- Include a $100 non-refundable deposit
- Include a copy of the child’s birth certificate
- Include a copy of the State of Illinois Certificate of Health Examination Form.
